SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/87-36 AND 50-328/87-36)

This refers to the Nuclear Regulatory Commission (NRC) special inspection

conducted by K. M. Jenison on May 26 - June 5, 1987. The inspection included a

review of activities authorized for your Sequoyah facility. At the conclusion

of the inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. These areas

included the Sequoyah Surveillance Review / Revision Program and Surveillance

Instruction ISI) performance. Within these areas, the inspection consisted of

selective examinations of procedures and representative records, interviews

with personnel, and observation of activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements. The violation involves inadequate channel calibration and

channel functional testing of the chlorine detection system. This inadequate

testing resulted in a failure to have an operable chlorine detection system in

accordance with Technical Specification 3.3.3.6. Accordingly, the violation,

references to pertinent requirements, and elements to be included in your

response are described in the enclosed Notice of Violation.

One unresolved item was identified concerning the adequacy of SI-218.2,

SI-270.2, and SI-166.40. During the course of this inspection, the Site

Director committed to re-evaluate these sis for technical adequacy. These

sis are discussed in paragraph 5 of the enclosed inspection report. The

re-evaluation for technical adequacy by TVA is to be completed within thirty

days of the receipt of this letter.

Credit for licensee corrective actions was afforded during this inspection to

l account for the SI review activities of the quality assurance organization and

the SI validation and verification group. These two organizations had identi-

fied weaknesses in the TVA SI review program described by TVA as admi.9 strative.

The NRC identified certain of these procedural attributes as necessary to

for each SI to ensure administrative discrepancies .do not invalidate the

. technical adequacy of each instruction.

In 'setordance with Section 2.790 of the NRC's " Rules of Practice,'.' Part 2,

'iitle 10, . Code.'of Federal Regulations, a copy of this letter, its enclosures,

and any reply will be placed in the NRC Public Document Room.

.The responses directed by this letter and its enclosure are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
