t APPENDIX A Limitorque Corporation Docket No. 99900100/86-01 NOTICE OF VIOLATION As a result of the inspection conducted on September 22-26,1986, and in accordance with Section 206 of the Energy Reorganization Act of 1974 and its implementing regulation of 10 CFR Part 21, the following violations were identified and categorized in accordance with the NRC Enforcement ?clicy (10 CFR Part 2, Appendix C), 49 FR 8583 (March 8, 1984):

1.

Section 21.31 of 10 CFR Part 21, dated May 31, 1984, states, in part, "Each... corporation... subject to the regulations in this part shall assure that each procurement document for a... basic component...

specifies, when applicable, that the provisions of 10 CFR Part 21 apply."

Contrary to the above, Limitorque did not pass on the requirements of 10 CFR Part 21 to WESCO (Purchase Order (P0) No.1825 dated April 18, 1986) for Rockbestos wire and to Barker Jennings Corpora. tion (P0 No. 3579 dated September 19,1986) for calibration services.

This is a Severity Level V violation (Supplement VII).

Section 21.51(b) of 10 CFR Part 21 dated May 31, 1984, states, in part, "Each individual, corporation... subject to the regulations in this part shall prepare records...to assure compliance with the regulations in this part."

Contrary to the above, Limitorque was unable to provide documented evi-dence that an adequate review and evaluation was performed for possible 10 CFR Part 21 reporting on Type SMB-000 motor operators containing wiring that can be damaged by the energization of the installed space heaters at determined by the following (86-01-02):

1.

A Georgia Power Company 10 CFR 50.55e report to the NRC dated March 20, 1986 described damaged (burnt) wiring on a number of safety related Limitorque motor operators.

2.

An NRC inspector at Vogtle Nuclear Power Plant on May 20, 1986 observed burnt wiring on a number of Limitorque operators.

This is a Severity Level IV violation (Supplement VII).
