APPENDIX A
Dresser Industries, Inc.
Docket No. 99900054/85-01
As a result of the inspection conducted on September 30-October 4, 19P5, and
in accordance with Section'206 of the Energy Reorganization Act of 1974 and
its implementing regulation 10 CFR Part 21, the following violation was
'
identified and categorized in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), 49 FR 8583 (March 8,1984):

Section 21.21 of 10 CFR Part 21 states in part:
"(a) Each individual, corpora-
tion, partnership... subject to the regulations in this part shall adopt appropriate
procedures to:
(1) provide for:
(i) evaulating deviations or (ii) informing
the licensee or purchaser of the deviation...(2)...of a facility, or activity,
or a basic component supplied...(i) relating to a substantial safety
hazard. .. (b)(1). ..a designated person shall noti fy the commission. . .(2). . .notifica-
tien shall be made to the Director, Office of Inspection and Enforcement, er to
the administrator of a regional office...(3) the written report shall include,

but need not be limited to...(vi)...in the case of a basic component which
contains a defect or fails to comply, the number and location of all such
'
components... supplied for one or more facilities...."
Contrary to the above, Dresser's Part 21 file no. 84-01 was not adequately
'
evaluated.
File no. 84-01 identified a problem with disc collar failure for
3707RA safety valves.
The file stated this problem was not reportable since
"The failure is an isolated case." However, a letter, dated 07/02/84, in the
same file states this failure has occurred twice since 1971.
Once in August
1981 at Toledo Edison / Davis-Besse and the second time at Diablo Canyon Unit #2
in May 1984
Infornation in file no. 84-01 also indicates that in May,1984, during blowdown
testing by Wyle Laboratories, Huntsville, Alabama, on the main stean safety
valves from Pacific Gas & Electric /Diablo Canyon 2 (Dresser valve type 3707PA),
valve S/N BN1741 lifted with simultaneous shearing of the disc collar / spindle
threads and cotter pin.
This occurred during the first actuation, at
approximately 1065 psig.
The disc and soindle deflected sidewise upon
closing and the disc became wedged between the nozzle seat area at G the bottom
of the disc holder.
The valve became mechanically jammed and could not open.
It also leaked severely because the disc was not seated properly.
As a result of this inadeouate Part 21 evaluation, Dresser has failed to
notify the NRC or its customers of this reportable Part 21 item (85-01-01).
This is a Severity Level !!! viglation (Supplement VIII.
