ENCLOSURE 3 NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-259, 50-E60, Browns Ferry, Units 1, 2, and 3

.and 50-296 License Nos. DPR-33, DPR-52, and DPR-68 The Nuclear Regulatory Commission (NRC) inspections conducted during various intervals between February 11 and May 23, 1986 identified a violation of NRC requirements.

In accordance with the " Generic Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is described below.

Criterion XVI of 10 CFR Part 50, Appendix B, requires that measures shall be esttblished to ensure that conditions adverse to quality, such as deficiencies, deviations, and nonconformances, are promptly identified and corrected. Section 17.2.16 of the TVA Quality Assurance Program Report implements these requirements and requires that the corrective action

reports become part of the plant quality assurance records.

It also states that "DQA makes followup reviews for implementation of the corrective action and reviews any reports generated by the action before the corrective action report is closed."

's Contrary to these requirements, on July 9, 1985 Nonconformance Report (NCR) BFNTDP7801 was closed by actions completed for Engineering Change Notice (ECN) L1845.

This issue involved failure to maintain separation between safety division I and safety division II circuits in that the recirculation pump flow monitoring circuit for each division was tied together on the same terminal block. The corrective action required degrading the circuits to non-safety and routing them in non-divisional trays ono conduits. The corrective action on the NCR was confirmed by the Browns Ferry engineering project manager as completed on Autjust 15, 1985.

However, this actico had, in fact, not been completed even though the rccords showed comp.etion (Sample 39).
