Trojan Nuclear Plant Summary of Operating Experience Docket No: 503344 Date: 4-8-86 Completed By: F. J. Ulmer Telephone 503-556-3713 OPERATIONS:
The plant began the month operating at full power and continued at
, full power until 1105 on March 4, 1986. At this time, power was reduced to 35% while maintenance work was performed on auxiliary feed-water valves to correct misaligned motor-to-valve stem couplings.
The coupling problem was corrected and system flow tests were performed to insure that adequate auxiliary feedwater flow was available. The plant returned.to. full power at 0800 on March 5. Full power continued until 1658, March 6; then power was' reduced to 55% to allow-repair of a cracked pipe and installation of additional supports in the 'A' train circulating water system at the 'A' condenser inlet connection.
Additional support was also added to the 'B' train. The plant returned to full power at 2115 on March 8.
During the month, the steam generator tube leakage into the 'C' steam generator increased from 250 gallons per day to about 350 gallons per day. The reactor coolant dose equivalent iodine remained steady at about 16% of the luCi/gm technical specification limit.
MAJOR SAFETY RELATED MAINTENANCE:
'A' train ESF outage for monthly preventative maintenance.-
'B' train ESF outage for monthly preventative maintenance.
CB-1 Control Room Emergency Ventilation repair / testing.
'A' and 'B' train auxiliary feedwater flow control valve coupling repairs.
Various breaker and MOV testing.
Time Response testing of the inputs to the Reactor Protection System.
MISCELLANEOUS MAINTENANCE:
Steam Dump (TCV-500c) bottom flange repair.
LICENSE CHANGES:
During the month of March, the following amendments-to Trojan's Operating License were incorporated:
l l
i
Trojan Nuclear Plant Summary of Operating Experience Page 2 Amendment No. 110 The amendment revises the Operating License and the Technical Specifi-cations to (1) revise the Surveillance Requirements for Axial Flux Difference, (2) delete the ACTION statement associated with reporta-bility of out-of-specification RCS chemistry in accordance with the revised Licensee Event Report (LER) rule of 10 CFR 50.72 and 50.73, (3) reflect a change to the fire pump diesel engine Surveillance Requirement, (4) correct inconsistencies in the RCS volume, (5) more clearly identify the low population zone, and (6) make editorial corrections.
Amendment No. 111 The amendment revised the Technical Specifications to clarify the surveillance requirements for the Containment Ventilation Isolation System (Section 4.9.9) by specifying which monitoring channels must be included in the system operability test.
Amendment No. 112 The amendment revised the Technical Specifications to add operability and surveillance requirements for the core exit thermocouples and the Reactor Vessel Level Instrumentation System (RVLIS).
Amendment No. 113 The amendment revised the Technical Specifications to reflect a change in the offsite and onsite Quality Assurance (QA) Organizations made to clarity the delineation of responsibilities, improve coordination between onsite and offsite QA groups, and improve operating indepen-dence for the onsite QA group.
Amendment No. 114 The amendment revises a number of areas of the Technical Specifications in order to clarify existing operability and surveillance requirements, to add new requirements or to correct errors that have been identified.
MISCELLANEOUS:
The NRC returned to the plant to continue their review of the ade-quacy of Trojan control room emergency ventilation system and testing methods. Additional changes were made to the system to correct its design and improve operability.
On March 27, members of the Trojan staff attended an NRC enforcement-meeting in Walnut Creek, California, regarding control. room emergency ventilation.
