i TROJAN NUCLEAR PLANT DOCKET NO: 50-344

SUMMARY

OF OPERATING EXPERIENCE DATE: 9-8-86 COMPLETED BY: F. J. Ulmer TELEPHONE: 503-556-3713 OPERATIONS:

The Trojan Nuclear Plant entered the month of August in Mode 1 at full power. On August 5, 1986, the plint tripped, restarted 10 hours1.157407e-4 days
0.00278 hours
1.653439e-5 weeks
3.805e-6 months
later, and returned to full power on August 6. Plant operation continued through the end of the month at full power, except for small power reductions (5% to 10%) to reduce high condenser back pressure during warm weather conditions.

On August 5, 1986 at 0953, the reactor tripped on a safety injection signal. The cause of the trip was a failure of a steam generator pressure transmitter while a periodic instrumentation test was in progress which made the 'B' steam generator pressure appear to be more than 100 psi lower than two of the other three steam generators. This condition satisfied the logic for a safety injection actuation and subsequent reactor trip. All safety systems and protection systems functioned in accordance with design requirements. The failed pressure transmitter was replaced.

At 1658 on the same day, the reactor was restarted and the turbine generator unit was synchronized to the grid at 2032. The plant power level was held at 40% power for steam generator chemistry concerns until the following morning at which time power was raised to 100%.

The state of Washington temporarily lifted the plant's permit to dispose of low-level radioactive waste at Hanford because a shipment of radioactive resin sent to Hanford for burial did not meet license requirements of the disposal site. When the disposal site unloaded the shipment, they discovered some contaminated water between the resin liner and the shipping cask. Trojan has taken corrective action to prevent reoccur-rences and the state of Washington reinstated Trojan's disposal permit.

MAJOR SAFETY-RELATED MAINTENANCE:

'A' train ESF outage occurred for monthly preventative maintenance.

'B' train ESF outage occurred for monthly preventative maintenance.

MISCELLANEOUS MAINTENANCE:

The technical specification required five year through bolt surveillance for the control building was completed satisfactorily.

Changed out all chemical and volume control system resin beds due to exhausted resins.

Completed modifying the burnable poison rod assembly tool to be used with installed spent fuel pool racks.

LICENSE CHANGES:

Amendment 118 The amendment deletes the requirement for shutdown if primary coolant iodine activity limits are exceeded for 800 hours0.00926 days
0.222 hours
0.00132 weeks
3.044e-4 months
in a 12-month period. In addition, this change revised the reporting requirements related to primary coolant specific activity levels. The amendment request was in response to Generic Letter 85-19.

! License Changes (Continued)'

I Amendment 119 The amendment deletes Technica1' Specification 3.4.10.1'.d which allowed Trojan to l- perform an evaluation to determine the consequences of continued operation with

! reduced structural integrity of ASME Code Class 1, 2, and 3 components. The amendment l request was in response to our February 10,' 1986 letter requesting your review of Technical Specification 3.4.10.1.d.

i !

MISCELLANEOUS:

l The plant was subject to an NRC audit of the residual heat removal and supporting service water / component coolant cooling water systems as part of an NRC program to ;

l Verify design adequacy and control of design changes.
