APPENDIX A Dietrich Standard Corporation Boulder, Colorado NOTICE OF NONCONFORMANCE An NRC inspection of Dietrich Standard Corporation was conducted on October 21-24, 1985, which was part of the MC's review of outage activities associated with Omaha Public Power District's Ft. Calhoun facility. During this inspection, Dietrich Standard's quality assurance and engineering activities in~ general and specifically those related to the safety-related flow element for the Ft. Calhoun Station component cooling water system were -

reviewed. This safety-related component, as procured (P0 No. 08005) for the Ft. Calhoun Station, was to be manufactured in accordance with the quality hssurance criteria specified in 10 CFR Part 50, Appendix B. Based upon the results of this inspection, it appears that certain of your activities were not conducted in accordance with NRC requirements. Nonconformances with these requirements are stated below.

1. Criterion XV of Appendix B to 10 CFR 50 requires that measures be established to control parts or components which do not conform to requirements such as applicable codes, standards, specifications, and criteria in order to prevent their inadvertent use or installation.

These measures shall include procedures for identification, documentation, segregation, disposition, and notification to affected organizations. Nonconforming items shall be reviewed and accepted, rejected, repaired or reworked in accordance with documented procedures.

Dietrich Standard QA manual Section 10, paragraph 10.2.4.1, and Section 13 paragraph 13.2.4 require, in part, that nonconformances be dispositioned by the Material Review Board (MRB), and that the MRB's disposition be documented on the nonconforming report, signed and dated by the responsible personnel.

Section 12, paragraph 12.2.1 requires in part that a nondestructive examination procedure be initiated when an assembly reaches the hold point in the manufacturing operation for such an examination.'

Contrary to the above, these procedures were not followed when Dietrich Standard QA Inspectors released the previously rejected and tagoed Ft.

Calhoun flow element assembly for a Nondestructive Examination (NDE) prior to the completion of a Nonconforming Material Report by authorized personnel. (85-01-01)

2. Criterion III of Appendix B to 10 CFR 50 requries, in part, that measures be established to assure that applicable regulatory requirements and the design basis for those components which are required to meet their functional goals under the effects of a postulated accident and to which this appendix applies, include provisions to assure that appropriate quality standards are specified and included in design documents. Also, design control measures shall provide for verifying or checking the adequacy of a design, such as design reviews.

3. Criterion XVII of Appendix B to-10 CFR 50 requires, in part, that sufficient records shall be maintained to furnish evidence of-activities affecting quality. Records shall also be identifiable and retrievable. Records shall include operating logs and the results of reviews, inspections, tests and monitoring of work performance. Inspection and test records shall identify the inspector, the type of observation, the results, the acceptability, and the action taken ir, connection with any deficiencies noted.

Dietrich Standard QA manual Section 20.2.1. requires, in part, that quality assurance maintain a master file for each order processed to ASME code requirements. The file is to be a repository for all documents pertaining to the design, materials, manufacturing, testing and examination identified on the invoice.

Contrary to the above, the Dietrich Standard Quality Assurance master file (No.12302) for a Northeast Nuclear Energy Company purchase of safety-related flow elements procured for the Millstone 3 Nuclear Power Plant did not include documentation for various nonconformance reports and subsequent production and inspection documents (NMPs) used to support a repair-described on Nonconformance Material Report No. 1703.
