NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on September 17-19, 1985, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follows:

A. Section 2 " Quality Assurance Program," of ANSI-N45.2-1977, states, in part, "...The program shall provide for indoctrination...of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained..."

Contrary to the above, as of September 19,1985(85-01-01):

1. The QA manual did not require QA program indoctrination training for ,

all Elgar personnel who were involved in the performance of quality i activities.

l 2. Records were not in evidence to indicate that applicable Elgar .

personnel were indoctrinated in regard to their QA program responsibilities, other than the QA and Manufacturing departments. i B. Criterion III, " Design Control", of Appendix B to 10 CFR Part 50, states, in part, "Meastres shall be established to assure that...The design basis....for... systems, and components... cre translated into specifications, drawings, procedures, and instructions..."

Contrary to the above, as of September 19, 1985 (85-01-02):

1. No design basis information, drawings, instructions, or specifications to assure that the design basis was adequately translated were in evidence which would indicate that the fuse holder 4

stud, with associated nuts and washers, was an allowable electrical i conductor on Elgar's static inverter model #253-1, filter panel assembly, drawing No. 643-389-40, 1

2. No assembly instructions or configuration specifications were in evidence to assure the mechanical interconnection of the fuse holder / shunt subassembly on Elgar's " filter panel assembly" was accomplished in accordance with applicable requirements. These -

components were identified as part numbers 857-PR4-00 (Shunt-400A-50MV),858-P26-60(fuseholder),and 943-390-20 (fuse Bus Bar),
