Grand Gulf Nuclear Station File 0260/0490/15521 RII: WPA 50-416/80-8 Response to NRC Site Inspection 80-08

Reference:

MAEC-80/72 AECM-80/104 Your Mr. W. P. Ang performed an inspection at the Grand Gulf Nuclear Station construction site on March 3-6, 1980. Your letter of April 1, 1980, transmitting the inspection results included a Notice of Violation for failure to follow pipe support installation and inspection procedures.

MP&L received your correspondence April 7, 1980, therefore, our response to the item of noncompliance would have been due April 27, 1980.

On May 16, 1980, your Mr. F. S. Cantrell telephoned to inform us that the NRC had not received a reply to the Notice of Violation. Our inquiry revealed that a response had not been subs!tted to your office. Immediate action was taken as a result of Mr. Ang's inspection as evidenced by the initiation of an investigation on March 7, 1980 which resulted in the gen- i eration of a potentially reportable deficiency on March 19, 1980 and the submission of a status report on April 18, 1980. The inquiry also revealed that this was an isolated case. All personnel involved have been admonished and reinstructed in the proper handling of NRC Inspection Reports.

Attached is MP&L's response to the Notice of Violation.

CORRECTIVE STEPS TAKEN AND RESULTS ACHIEVED An investigative inspection of seventy-seven (77) pipe hanger installations war conducted to determine the extent of the problem. This inspection revealed that twenty-three (23) of the inspected hangers exhibited speci-fication nonconformances. The Constructor br a issued nonconformance docu-ments to effect specific corrective action.

As a result of the Constructor inspection, MP&L initiated a potentially reportable deficiency and notified the NRC, Region II, on March 19, 1980.

A status report was submitted to Region II on April 18, 1980 by letter number AECM-80/82.

The Constructor has issued MCAR 66 to document the generic problem. An inspection program has been initiated which includes a reinspection of all previously accepted hanger installations which are accessible. Ac-ceptability of hangers which are not accessible will be based on an analysis of our inspection results. Nonconformances found as a result of this in-spection shall be documented.

CORRECTIVE STEPS TAKEN TO AVOID FURTHER NONCOMPLIANCE The Constructor revised the Qua'lity Control Instruction pertaining to the installation of pipe hangers and issued the instruction on April 16, 1980. Quality Control and Field Engineering personnel received training on installation and inspection procedures during the period April 21-28, 1980.

DATE TO ACHIEVE FULL COMPLIANCE Full compliance was achieved on May 1, 1980.
