s ATTACHMENT TO AECM-79/138 A. NRC Item 416/79-21-01 "LPCS Globe Valve F012 Incorrectly Installed" (1) Corrective Steps Taken and Results Achieved Bechtel Quality Assurance issued QAR F-152 on August 28, 1979 to Construction and QAR F-154 on August 30, 1979 to Project Engineering for, in part, determination of cause, formulation and implementation of corrective action, and formulation and implementation of actions taken to preclude recurrence. The fourteen-inch globe valve, Q1E21F012, could not be installed in compliance to the drawing because of an interference. As a result of conversations between Field Engineering and Project Engineering and conversations and correspondence between Project Engineering and General Electric, the subject valve was installed in its present state. The failure to comply with programmatic requirements occurred when the valve was installed prior to the drawings being revised.

(2) Corrective Steps Taken to Avoid Further Noncompliance Involved personnel have been verbally reinstructed in programmatic requirements. Drawing 9645-M-1350A has been revised to reflect installation as has Drawing 9645-M-1087.

Therefore, the valve, as installed, is currently per the isometric drawing and the piping and instrumentation diagram.

(3) Date to Achieve Full Compliance Full compliance was achieved on November 28, 1979.
