No. 12 Containment Fan Coil Unit - Inoperable.

This report was initiated by Incident Report 82-080.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On April 10, 1982, during surveillance testing, the Control Room Operator discovered that the service water flow to No. 12 Containment Fan Coil Unit (CFCU) was less than the 700 GPM required. No. 12 CFt] was declared inoperable, and at 1211 hours0.014 days <br />0.336 hours <br />0.002 weeks <br />4.607855e-4 months <br />, Action Statement 3.6.2.3.a was entered.

Redundant CFCUs and both Containment Spray Systems were operable throughout the occurrence.

j DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Flow Control Valve 12SW223 had stuck in the closed position, resulting in low service water flow to the CFCU. The binding of type SW223 valves is e.pparently associated with the accumu-l lation of scale and corrosion on the valve stems, resulting from the CFCUs being idle between monthly surveillance runs.

ANALYSIS OF OCCURRENCE:

The CFCUs operate in conjunction with the containment spray systems to remove heat and radioactive contamination from the containment atmosphere in the event of a design basis accident.

Operability of the equipment is necessary to insure offsite radiation dose is maintained within the limits of 10CFR100.

As noted, containment cooling capability was provided by the redundant equipment; no undue risk to the health or safety of the public was therefore involved in this occurrence. The occurrence constituted operation in a degraded mode permitted by a limiting condition for operation and is reportable in accordance with Technical Specification 6.9.1.9b.

Technical Specification 3.6.2.3.a requires:

With one group of containment cooling fans inoperable, and both containment spray systems operable, restore the inoperable group of cooling fans to operable status I

within 7 days, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION:

Valve 12SW223 was exercised, and No. 12 CFCU was satisfactorily tested. The unit was declared operable, and at 1540 hours0.0178 days <br />0.428 hours <br />0.00255 weeks <br />5.8597e-4 months <br />, April 10, 1982, Action Statement 3.6.2.3.a was terminated.

Due to recurrent problems of this type, daily operation of CFCUs in low speed was commenced in December 1982 (per revision of Operation Directives 40-46). No further problems have been noted since that time. Running the CFCU results in cycling of the associated control valves and has apparently eliminated the problem with the sticking of these valves.
