APPENDIX A Cooper Energy Services Docket No. 99900317/86-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on July 28-August 1,1986, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribec by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Section IV of Standards Numbering and Handling, Procedure No. SA-1 states, in part, " Superseded sheets [of Engineering and Materials Standards] should be rernoved and destroyed. . . ."

Contrary to the above, out of date procedures and an out of date procedures index were found in the heat treat area office. The out of date heat treat procedures were the subject of Nonconformance B.1. identified in the previous inspection which was performed during March 1984(86-01-01).
