APPENDIX A Nuclear Packaging, Incorporated Docket No. 99901047/86-02 NOTICE OF NONCONFORMANCE During a special inspection conducted June 21-26, 1986, the implementation of the Nuclear Packaging, Incorporated (NUPAC) Quality Assurance (0A) Program at the Federal Way, Washington facility was reviewed with respect to the nondestructive testing (NDT) performed during fabricatica of the two Model 125-B shipping casks. Certificate of Compliance (C of C) Nymber 9200, dated April 11,1986, requires that the Model 125-B shipping package (cask) be fa-bricated in accordance with the information and specifications contained in the application for cask certification. NUPAC's application to the NRC was dated June 14, 1985 and supplemented on October 31 and November 22, 1985, and on February 11, 1986. Specifically, C of C Number 9200 requires, in part, that the packaging be constructed in accordance with NUPAC Drawing No.

X-101-100, sheets 1 through 6, Revision H, dated February 10, 1986. The NDT requirements for the Model 125-B casks are specified on this drawing.

As a result of the inspection, it appears that certain activities at the NUPAC facility were not conducted in accordance with commitments made to the NRC.

In accordance with Section 206 of the Energy Reorganization Act of 1974, the following nonconformance was identified in accordance with the NRC Enforcement Policy, 10 CFR Part 2, Appendix C.

Paragraph 71.119 of Subpart H to 10 CFR Part 71 requires, in part, that the licensee establish measures to assure that special processes including nendestructive testing are controlled and accomplished by qualified personnel using qualified procedures.

Section 3.3.2 of LT-26, " Outer Cask Hydrostatic Pressure Test," Revision 0, dated November 6,1985, states, in part, that during the hydrostatic pressure test a recording chart capable of recording test pressures during the time duration of the test.

Contrary to the above, LT-26 was not followed by Olympic Northwest Industries in that pressure recording charts were not used during the outer cask hydro-l static pressure tests on either Model 125-B shipping cask.
