This refers to the inspection conducted by Messrs. J. Craig, R. Cilimberg, P. Prescott, J. White, B. Clark, and Ms. C. Abbate on June 21-26, 1986, at your facility in Federal Way, Washington and to the discussions of our inspection findings with members of your staff at the conclusion of the inspection.

This special inspection was conducted as a result of unresolved items identified during the inspection performed May 5-8, 1986. These unresolved items involved nondestructive testing (NDT) associated with the two NUPAC Model 125-B casks.

The purpose of this special inspection was to review NDT quality records associated with these casks. The inspectors reviewed NDT test procedures, NDT personnel cualifications and NDT test results for tests performed on the Model 125-B casks. The areas examined during the inspection and our findings are discussed in the enclosed report.

The adequacy of leak testing and the qualification of the individual performing leak testing on the Model 125-B casks was specifically identified as an unresolved item in Inspection Report 99901047/86-01. Based upon the documents reviewed during this inspection, NDT activities, including leak testing, test procedures and tests were adequately performed by qualified personnel. The test results were acceptable and satisfied the NDT requirements specified in NRC Certificate of Compliance (C of C) Number 9200.

During the inspection, an indication on two radiographs was initially classified as incomplete penetration. This indication was located in the longitudinal weld of cask 2. Subsequent to the inspertion, a team of independent NDT Level III examiners, examined the radiographs for the NRC and additional information provided by a computer enhancement of the radiographs. The indication on the radiographs has been determined to be a slag inclusion which is acceptable as defined by the criteria of NRC C of C Number 9200 issued for the NUPAC Model 125-B cask design. Refer to the Addendum to the enclosed report for additional details of this review.

This item and the findings concerning radiographs illustrate a weakness in your activities to ensure that tests performed for NUPAC by other contractors are properly conducted in accordance with approved procedures. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

Please provide us within 30 days from the date of this letter a written statement containing: (1) a description of the steps that have been or will be taken to correct these items: (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventative measures were or will be completed. Consideration may be given to extending your response time for good cause shown.

The responses requested by this letter are not subject to the clearance procedures of the Office of Management and Budget as reouired by the Papemork Reduction Act of 1980, PL 96-511.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed report will be placed in the NRC's Public Document Room.
