APPENDIX A Combustion Engineering, Inc.

Docket No. 99900401/85-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 18-22, 1985 it appears that certain of your activities were not conducted in accordance with NRC requirements.

1. CE QADP 5.2, Section 6.0, " Computer Code Error Reports," states, in part, "The cognizant engineer distributes error reports to each of the managers on the distribution list using the Computer Code Error Notification Form, which includes a Computer Program Error Notification Response Receipt on the back of the form. The manager in turn circulates the error notice within his group. The users in his group sign and date the form to acknowledge receipt of the notice."

Contrary to the above, one manager and two supervisors on the distribution list for the CESEC computer code had no documented evidence (Computer Code Error Notification Form) that they had circulated the latest three CESEC computer code error report's to the users within their groups for-signoff and dating. 2. CE QADP 5.2, Section 1.2, states, " Documentation of design analyses is sufficiently detailed in purpose method, assumptions, design input, references and units so that a person technically qualified in the subject can review and understand the analysis and verify the results without recourse to the originator."

Contrary to the above, the St. Lucie 1 Reactor Vessel Level Monitoring System (RVLMS) water drainage calculation (19367-LOCA-026) and the RVLMS Phase III Prototype Testing procedure (00000-ESE-344) incorporated supplemental information which was not properly referenced. The RVLMS water drainage calculation included the splash guard area which was incorporated from another calculation. The reference was made to the author of the calculation and not to a specific calculation. Further, this supporting calculation was not provided in the reference list for the above mentioned calculation. Also, the RVLMS Phase III Prototype Testing Procedure specified a range of depressurization rates, but no specific reference for these depressurization rates was provided.
