Gentlemen:

( SUBJECT: REPORT N05. 50-327/85-46 AND 50-328/85-46

l This refers to a special NRC team inspection conducted during the period of

December 9-13, 1985, of activities authorized by NRC Operating License

( Nos. DPR-77 and DPR-79 for your Sequoyah facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff iden -

,

tified in the enclosed inspection report. This report replaces the previously

! issued Inspection Report Nos. 50-327/85-46 and 50-328/85-46, dated January 22,

!

1986, because that report was inadvertently issued prior to completion of

management review and approval.

The inspection focused on selected areas of Sequoyah operational readiness

addressed in Mr. C. Dean's November 1, 1985 submittal in response to

Mr. W. Dircks' September 17, 1985 10 CFR 50.54(f) letter. Specific areas

examined during the inspection are identified in the report. Within these areas,

the inspection consisted of selective examinations of procedures and representa-

tive records, interviews with personnel, and observation of activities in

progress.

The inspection findings indicate that certain activities violated NRC require-

ments. The violations, references to pertinent requirements, and elements to be

included in your response are presented in the enclosed Hotice of Violation.

Your attention is invited to unresolved items identified in the inspection

report. These matters will be pursued during future inspections.

The responses directed by this letter and the enclosures are not subject to the

clearance procedures of the Office of Management and Budget issued under the

Paperwork Reduction Act of 1980, PL 96-511.
