This report is submitted in accordance with Sequoyah unit 1 Technical Specifications 3.7.12 and 6.9.2.

l SEQUOYAH NUCLEAR PLANT SPECIAL REPORT 83-04 UNIT 1 a

Event Description Mechanical penetration door A196 in the auxiliary building (elevation 749) was breached for a period greater than seven (7) days. The penetration was breached by Operations at 0800 (C) on 10/31/83 and returned to service on 11/08/83 at 1100 (C). A roving firewatch was established as required by technical specification action statement 3.7.12.1.

Probable Cause The cause of the event was due to the failure of the shift engineer /firewatch to recognize the expiration date for breach.

Corrective Action Firewatch patrol personnel have been instructed to check expiration dates on all breaches and notify the shift engineer prior to the expiration date.
