SEQUOYAH NUCLEAR PLANT (SQN) - UNIT 2 - FACILITY OPERATING LICENSE DPR TECHNICAL SPECIFICATION (TS) 3.7.12 - SPECIAL REPORT 95-05 The enclosed special report provides details concerning a Unit 2, auxiliary building pipe sleeve penetration that is nonfunctional as a fire barrier. This fire barrier was intentionally removed from service to facilitate repair work on a flexible-shaft remote valve operator.

The pipe sleeve penetration was nonfunctional as a fire barrier for a time period greater than the TS allowable timeframe.

This report is being submitted in accordance with TS 3.7.12 Action Statement (a).

1 Descriotion of Condition On June 21,1995, the auxiliary building pipe sleeve, Mark No. 210, located on Floor Elevation 669 was breached. This fire barrier penetration was intentionally removed from i

service to support the planned repair of a flex-shaft remote valve operator for Valve No SQN-2-VLV-062-0541. The pipe sleeve provides access between the penetration room and the Unit 2 auxiliary building pipe chase on Floor Elevation 669. The penetration has been breached in excess of the technical specification (TS) allowable timeframe of seven days. This condition is being reported in accordance with TS Action Statement 3.7.12(a). ,

Cause of Condition j It was determined that a flex-shaft remote valve operator required repair to allow proper valve operation. To facilitate this activity, the remote operator was removed from the pipe sleeve, resulting in the fire barrier penetration being breached. As repair work progressed, it was determined that the work package did not contain sleeve sealing information because l the type of penetration seal was not specified on the design drawing. Drawing Deviation  :

DD-950142 was initiated to revise the drawing to include the seal information. The work document was revised to incorporate penetration sealing instructions and has been r

rescheduled.

Corrective Action In accordance with TS Action Statement 3.7.12(a), a roving fire watch was established after the fire detectors on one side of the penetration were verified operable. The roving fire .

watch will be maintained until the penetration fire barrier is reestablished. The fire barrier [

will be reestablished after the completion of the repair work associated with the remote valve operator. Repair work has been rescheduled and is anticipated to be complete in August 1995.
