Dear Mr. Keppler:

Toledo Edison acknowledges receipt of your August 20, 1975 letter and enclosure referencing an apparent noncompliance with NRC regulations.

This item resulted from the site inspection conducted by our office on July 29-31, 1975.

Toledo Edison offers the following information regarding the apparent non-U compliance, including corrective action and steps taken to avoid further occurrences.

Item A-2: Contrary to 10 CFR Part 50, Appendix B, Criterion XIII,

, construction scaffolding was found restine on the stem of decay heat removal pumo discharge valve No. DH44, and the bonnets of by-pass line valves B59-4 and B59-3 were being supported by wire fastened to line 10B-HCC-50. (Report details, Section 1, Paragraph 2).

The following corrective action has been taken on the items outlined in the report details, Section 1, Paragraph 2.

a) The construction scaffolding found resting on the stem of valve DH44 has teen removed.

b) The wire supporting.the bonnets of valves B59-4 and B59-5 from line LOB-HCC-50 (non-Q) has been removed.

c) The installation of hanger 33C-GCB-7-H8 has been completed.

d) No response is offered to this item as it was corrected during the inspection.

c) Hanger 40-HCC-34-H2 (non-Q) has not been cut and is installed as x_- ,

shown on the approved hanger detail sketch. However, a small pipe hanger (non-Q) in the vicinity of this hanger had been cut due to a piping interference and this hanger has since been removed.

f) The stainless steel studs have been removed from the pipe tunnel, identified and stored in the warehouse.

g) Hanger 33B-GCB10-H15 has been identified as having flame cut holes in the hanger base plate, and will be repaired at a later date. ,

To prevent recurrence, the Bechtel Field Construction Manager has written a letter to all contractors instructing them to stress the importance of taking care of permanent plant equipment. In this instance, the Grinnell Project Manager has cautioned the Grinnell Piping Superintendents not to allow these practices to be used in the future. TECo QA and Bechtel QA/QC have also increased their surveillance of housekeeping practices.

ITT Grinnell is writing an inspection instruction and procedure to include inspection for flame cut holes. Hangers found having flame cut holes will be modified through approved repair procedures.
