Submitted with this letter is a correction to the dose history of Timothy J. Chambers, SSN 281-70-3274. During the period 1980 - 1997, the dose records for Mr. Chambers were incorrectly recorded using the SSN 281-70-3279.

Enclosed are copies of the reports (on NRC Form 5) furnished to the individual concerned and showing the correct SSN. This information is provided in accordance with 10CFR20.2206.
