Dear Mr. Dyer:
In accordance with 10 CFR 55.31(a)(2), the Davis-Besse Nuclear Power Station (DBNPS) submits the original and two copies of the preliminary Personal Qualification Statement -
Licensee (NRC Form 398) and approved Certification of Medical Examination by Facility Licensee (NRC Form 396) for the operator license examination scheduled the week of September 13,1999. Preliminary applications for Reactor Operator (RO) are being submitted for the following license candidates:
