SUBJECT:
DAVIS-BESSE INSPECTION REPORT 50-346/99004(DRP)
Dear Mr. Campbell:
On May 13,1999, the NRC completed an inspection at your Davis-Besse site. The enclosed
report presents the results of that inspection.
Operation of the plant was characterized by conservative decision making. Of particular note
was the initiative taken to plan a mid-cycle maintenance outage and then to start the outage
2 weeks early to address an upward trend in reactor coolant system (RCS) leakage. Plant
personnel prepared for ar,d conducted the outage in an organized and structured manner.
Operator performance during the reactor shutdown for the outage, during RCS deep drain
activities, and while starting up the reactor was error free. However, the inadvertent actions of
<
a worker which led to two circuit breakers being out-of-position for a period of approximately
7 hours8.101852e-5 days
0.00194 hours
1.157407e-5 weeks
2.6635e-6 months
and configuration control errors during the venting of reactor coolant pump seals and
during a calibration test for a reactor protection system temperature module detracted from the
otherwise good performance during this inspection period.
Based on the res'Jlts of this inspection, the NRC has determined that two violations of NRC
requirements occurred. These violations are being treated as Non-Cited Violations (NCVs),
consistent with Appendix C of the Enforcement Policy. These NCVs are described in the
subject inspection report. If you contest the violations or the severity level of these NCVs, you
should provide a response within 30 days of the date of this inspection report, with the basis for
your denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk,
Washington, DC 20555-0001, with copies to the Regional Administrator, Region 111, and the
Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington,
DC 20555-0001.
