October 31, 1986 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PN0-II-86-83 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verifi-cation or evaluation, and is basically all that is known by the Region II staff on this date.

SUBJECT:

11 IN-CORE DETECTORS 'JNACCOUNTED FOR Tennessee Valley Authority (TVA) notified Region II on October 24 that it could not dit:rmine the exact locatiJn of 11 of 23 in-core detectors which have been withdrawn from ths core and placed with- radioactive waste at Sequoyah since 1980. The detectors each contain .004-gram of high enriched uranium-235.

TVA checked on these detectors after five fission detectors were found to be missing at the t Browns' Ferry plant (PN0-II-86-75 and PNO-II-86-75A, issued October 15 and October 20). )'

Upon receiving this notification, Region II dispatched inspectors to Sequoyah, where they monitored TVA's follow-up actions during an inspection on October 29 and 30.

~ TVA believes ,the detectors are in the on-site radioactive waste area, but is conducting a search to determine if that is the case. The licensee is to notify NRC of its findings on or before November 3. TVA did not determine that these items were missing during its most recent inventory in March _of this year.

Both TVA and Region II have responded to media inquiries. TVA issued a press release today.

The State of Tennessee has been informed.
