s ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric Power Company Docket Nos. 50-338, 50-339 North Anna License Nos. NPF-4, NDF-7 During the Nuclear Regulatory Commission (NRC) inspection conducted on April 3-7, 1989, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforce-ment Actions," 10 CFR Part 2, /ppendix C (1989), the violation is listed below:

10 CFR Part 50, Appendix B, Criterion XVI states that measures shall be established to assure that conditions adverse to quality such as deficiencies, deviations, defective material and nonconformances are

, promptly identified and corrected.

Contrary to the above, the licensee feiled to identify conditions adverse to quality as follows:

(a) The main control panel wiring and terminal block section had not been cleaned for an extended period of time resulting in a buildup of dust which could affect the integrity of the wiring.

(b) The Nuclear Instrumentation System Cabinets (two per Unit) which

' contain the excore neutron flux instruments in the main control room contained one un-terminated multi-conductor control cable with bare (un-taped) conductors.

(c) A loose floor board in the control room constituted a deficiency because it could cause a person to fall into the wiring section of the open-back main control board.

(d) In nine panels in the main control room the bottom sealing boards were broken or missing.

This violation applies to Units 1 & 2, and is Severity Level IV, Supplement I Pursuant to the provisions of 10 CFR 2.201, Virginia Electric Power Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Comission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC l Resident Inspector, within 30 days of the date of the letter transmitting this 1 Notice.
