I ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power Company Docket Nos. 50-338 and 50-339 North Anna License Nos. NPF-4 and NPF-7 During the Nuclear Regulatory Commission (NRC) inspection conducted on January 16 - February 23, 1988, one violation of NRC requirements was identified. In accordance with with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:

Technical Specification 6.8.1.a requires written procedures be established, implemented, and maintained covering procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Contrary to the above:

On January 16, 1988, the Reactor Coolant System (RCS) level standpipe scale was installed without a procedure to provide the necessary guidance and verification to ensure proper installation.

This lack of procedural requirements resulted in the standpipe scale being installed approximately six inches too high which in turn resulted in the RCS vessel level being maintained six inches higher than expected. This higher level contributed to an inadvertent RCS leak of approximately 1000 gallons to the containment sump.

On January 23, 1388, mechanical maintenance personnel performed maintenance on the wrong flange on the Unit 1 "B" RCS Loop RTD bypass line.

The maintenance was performed on 1-RC-R0-1 instead of 1-RC-FE-1491 as specified in Mechanical Maintenance Procedure MMP-C-P-4 which referenced Work Order 5900070293.

This failure to follow a maintenance procedure resulted in an inadvertent RCS leak of approximately 1000 gallons to the containment sump.

This is a Severity Level V violation (Supplement 1) and applied to Unit 1 only.

Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, North Anna, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include [for each violation]:

(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the htSO4080257 880331

Where good cause is shown, consideration will be given to extending the response time.

If an adequate reply is not received within the time specified in inis Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
