ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power Company Docket Nos. 50-338 and 339 North Anna Units 1 and 2 License Nos. NPF-4 and NPF-7 During the Nuclear Regulatory Commission (NRC) inspection conducted on November 7, 1986 - January 11, 1987, violations of NRC requirements were identi-fied. The violations involved a failure to file a report for a contaminated person, an inadequate procedure, and a failure to take prompt corrective action to evaluate a potential prtblem with safety related equipment. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1986), the violations are listed below:

A. Technical Specification 6.11, Radiation Protection Program, requires that procedures shall be prepared consistent with the requirements of 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposures.

Section 4.2.2 of Health Physics Procedure 3.1.7 requires that Rubidium-88 contaminatior, be recorded on the clothing contamination section of form H.P.

Contrary to the above, Heath Physics Procedure 3.1.7 was not adhered to since a report was not filled out for a technician contaminated with Rubidium-88 on December 12, 1986, at approximately 3:00 p.m. (EST).

l This is a Severity Level IV violation (Supplement 1) and applied to both units.

B. Technical Specification 6.8.1.a requires written procedures be established, implemented and maintained covering the startup operation and shutdown of the Chemical and Volume Control System.

l Contrary to the above, on November 26, 1986, 2-0P-8.1 " Chemical and Volume i Control System" was inadequate in that this procedure allowed placing of 2-CH-P-1A charging pump into service with service water isolated which resulted in the overheating of the bearings of the A charging pump speed increaser and subsequent inoperability of the A charging pump.

l This is a Severity Level V violation (Supplemental 1) and applied to Unit 2.

Virginia Electric and Power Company 2 Docket Nos. 50-338 and 339 North Anna Units 1 and 2 License Nos. NPF-4 and NPF-7 C. 10 CFR 50, Appendix B, Section XVI, states the following:

"XVI Corrective Action - Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected. In the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition.

The identification of the significant condition adverse to quality, the cause of the condition, and the corrective action taken shall be documented and reported to appropriate levels of management."

Contrary to the abnve, appropriate action was not taken by the licensee to ensure that safety related equipment in the Main Steam Valve House (MSVH) had the proper analysis for Environmental Qualification (E0) until December 31, 1986, even though they had been notified of the potential for the problem by Westinghouse in June of 1984 and by the NRC in Inspection and Enforcement Information Notice (IEIN) 84-90, dated December 7,1984. The licensee's corporate engineering department performed an analysis in October of 1986 but appropriate actions were not taken until December 31, 1986.

This is a Severity Level V violation (Supplement 1) and applied to both units.

Pursuant to the pr visions of 10 CFR 2.201, Virginia Electric and Power Company is heraby requireo to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (for each violation): (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
