Virginia Electric and Power Company Docket Nos. 50-338 and 50-339 i North Anna Units 1 and 2 License Nos. NPF-4 and NPF-7 .

Ouring the Nuclear Regulatory Commission (NRC) inspection conducted on July 16 -

August 19, 1958, one violation of NRC requirements was identified. In accordance with with the "General Statement of Policy and Procedure for NRC ,

Enforcement Actions," 10 CTR Part 2 Appendix C (1987), the violation is listed l r

below:  !

A. Technical Specification 6.8.1.a requires written procedures be estab-lished, implemented, and maintained covering access to the containment as i documented in Appendix A of Regulatory Guide 1.33. Revision 2, February (

Administrative Procedure (AP) 20.9, Containment Ingress and Egress,  :

section 7, his a note stating "Each department will complate the [

applicable sections of attachment 1 as the steps are completed". Step 1 7.14 of AP 20.9 requires the health physics technician to determine the stay time using the sling psychrometer and document the wet and dry bulb temperatures and actual stay time on Attachment 1 of the procedure.

a. Contrary to the above, the inspectors identified that the licensee had not documented the wet bulb temperature or calculated the stay  ;

time for the containment entries made on August 8, 1938, at 0924 and ,

1618. This determination was nade based of, a review of Attachment 1 l of AP 20.9 used fo: the containment entries.  !

b. Contrary to the above, during a containment entry made on August 9 I the licensee f ailed to complete Attachment 1 of AP 20.9 as required l by the procedure.

This is a Severity Level V violation (Supplement 1) and applied to Unit 1 only. I Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company l 15 hereby required to submit a written statement or explanation to the Nuclear t l Regulatory Comission, ATTN: Document, Control Desk, Washington, OC 2C555, with  ;

I a copy to the Regional Acminist,rator, Region !!, and a copy to the NRC Resident, f l Inspector, North Anna, within 30 days of the date of the letter transmitting I this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include (for each violati'on): (1) admission or denial of the violation, (2) the reason for the violat,ien if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps whicn will be taken to avoid further violatters, and (5) the date when full i
