NOTICE OF VIOLATION

Virginia Electric and Power Company Docket Nos. 50-338, 50-339

North Anna Units 1 and 2 License Nos. NPF-4, NPF-7

During the Nuclear Regulatory Commission (NRC) inspection conducted on

November 20 - December 18,198'/ , violations of NRC requirements were identi-

fied. The violations involved a failure to follow the . requirements of a

radiation work permit and failure to properly implement post maintenance

testing. In accordance with the "General Statement of Policy and Procedure

for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations

are 1isted below:

Technical Specification (T.S.) 6.11 requires that procedures for personnel

radiation protection shall be prepared consistent with the requirements ,

of 10 CFR Part 20 and shall be approved, maintained, and adhered to for .

all operations involving personnel radiation exposure. Health Physics

'

procedure 5.3.20 "Initiating, Using, Extending and Terminating a Radiation

,

Work Permit (RWP)" requires a radiation worker to comply with the require-

ments, instructions, and precautions of the RWP.

Contrary to the above, on November 24, 1987, several operations personnel

failed to follow the requirements of RWP 87-3156, which required a whole

body frisk after leaving the area, in that they failed to perform a whole

body frisk at the designated frisking station.

This is a Severity Level IV violation (Supplement V). (Unit 1 only)

Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company

is hereby required to submit a written statement or explanation to the Nuclear

Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555,

with a copy to the Regional Administrator, Region II, and a copy to the NRC

Resident Inspector, North Anna, within 30 days of the date of the letter

transmitting this Notice. This reply should be clearly marked as a "Reply to a

Notice of Violation" and should include [for each violation]: (1) admission or

'

denial of the violation, (2) the reason for the violation if admitted, (3) the

corrective steps which have been taken and the results achieved, (4) the '

corrective steps which will be taken to avoid further violations, and (5) the

date when full compliance will be achieved. Where good cause is shown, con-

sideration will be given to extending the response time. If an adequate reply
