10 CFR.Part 2, Appendix C (1986), the violation is listed below:

110 CFR 50 Appendix B_ Criteria VI states the following:

" Measures.shall be established to control the issuance of documents, such'as instructions, procedures. . and drawings, -including changes thereto, which prescribe all activities affecting quality. These-measures shall assure that documents, including changes, are; reviewed for adequacy and approved for release by authorized personnel and are distributed to and used at the location where the prescribed activity is performed. Changes to documents shall be reviewed Land approved by the same organizatior.s that performed the original ~ rev.iew and . approval unless the applicant designates another responsible organization."

Contrary to the above, adequate measures were not established to assure the adequacy of the issuance of Technical Specification (TS) amendments as shown in following examples. (1) On August 27, 1986, amendments 84 for Unit 1 and 71 for Unit 2 were incorporated into the station TS but these amendments as, incorporated by the licensee, contained information different from that approved by the NRC: on August 25, 1986.' (2) On October 22, 1986, the

licensee final issuance of amendment 71 for Unit 2 contained minor typo-i graphical errors and an omission which changed the technical meaning of information which had been approved by the NRC on August 25, 1986.

results achieved, (4) the corrective steps which will be taken to avoid further violations, and '(5) the date.when full compliance:will be achieved. Where good cause is shown, consideration will be given to extending the response time.
