NOTICE OF VIOLATION I

Virginia Electric and Power Company Docket No. 50-338 North Anna License No. NPF-4 During the Nuclear Regulatory Comission (NRC) inspection conducted on August 17-21 1987, a violation of NRC requirements was identified. The viola- '

tion involved a failure to identify and correct damaged equipment and; failure to maintain record of a modification. In accordance with the " General State-ment of . Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the v.iolation is listed below:

10 CFR Part 50, Appendix B, Criterion XV states, in part, that measures shall be established to control parts or components which do not conform to requirements. Nonconfonning items shall be reviewed and accepted, rejected, repaired or reworked in accordance with documented procedures.

The UFSAR, Section 17.2.2 states that the quality assurance program for the operational phase is described in Topical Report VEP-1 and conforms to 10 CFR 50, Appendix B. VEP-1-4A, Section 17.2.15 requires that the individual discovering a nonconformance (including during fabrication and erection) identifies, describes and documents the nonconformance on a Nonconformance Report. Should the disposition of a nonconformance require the rework of structures such rework is reinspected. The rework is documented and becomes a part of station records.

Contrary to the above requirement a Nonconformance Report was not generat-ed to disposition nonconformances as described in the following two examples.

1. The NRC inspector, while inspecting a sample of terminations, noticed that three terminations were damaged by a short-circuit that had occurred in the past. The damage consisted of melted lugs and burned insulation. The wires on two of the terminal points are in the main steam line trip valve TV-MS101A control circuit (125VDC), which is safety-related. The terminal points in question are at the main control panel 1-EI-CB-05 field wiring terminal blocks.
2. Multiplexer support rack 1-EI-CB-300 was fabricated in the site shop as part of Design Change 81-S20A, NUREG-0696 Short Term I&C Project-Remote Multiplexer Installation. After the rack was built, it was found that the power supply cabinet did not fit into the rack because the rack was slightly too narrow. A Nonconformance Report was not generated to disposition the nonconformance.

Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including (for each violation): (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
