APPENDIX A NOTICE OF VIOLATION Portland General Electric Company Docket No: 50-344 Trojan Nuclear Plant License No: NPF-1 During an NRC inspection conducted from October 18 through November 21, 1987, two violations of NRC requirements were identified. In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations are listed below: CFR 50, Appendix B, Criterion V, states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance witn these instructions, procedures or drawings."

Plant periodic operating test, P0T-28-1, Core Quadrant Power Tilt Ratio Verification, Revision 8, specifies in part that QPTR values exceeding 1.020 after calculations using nuclear instrumentation detector voltages shall be confirmed by performing c nuclear instrumentation detector current calculatio Contrary to the above, during three instances of required technical specifications surveillance testing performed on Saptember 12, September 19, and September 26, 1987, (fetector current calculations were not performed after detector voltage calculations showed QPTR values

exceeding 1.0 This is a Severity Level IV violation (Supplement I). Technical specification 4.10.1.2 states, "Each full length rod not fully inserted shall be demonstrated operable by verifying its rod drop time to be less than or equal to 2.2 seconds within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> prior to reducing the shutdown margin to less than the limits of Specification 3.1.1.1."

Contrary to the above, during low power physics testing conducted in July 1985, September 1984, and July 1983, rod drop time measurement was perfornied for only one full length control rod (at location K-06) within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> prior to reducing the shutdown margin to less than the limits of Specification 3.1. This is a Severity Level IV violation (Supplement I).

I-2-Pursuant to the provisions of 10 CFR 2.201, Portland General Electric Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspector, Trojan Nuclear Plant, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a

" Reply to a Notice of Violation" and should include for each violation: (1) '

the reason for +.he violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Consideration may be given to extending the response time for good cause show FOR THE NUCLEAR REGULATORY COMMISSION
