APPENDIX A NOTICE OF VIOLATION Portland General Electric Company Docket No. 50-344 121 S. W. Salmon Street License No. NPF-1 Portland, OR 97204 As a result of an inspection conducted during the period of May 19 - June 12, 1986 and in accordance with the NRC Enforcement Policy, 10 CFR Part 2, Appendix C, the following violation was identified.
10 CFR 50, Appendix B, criterion XVI and the Trojan Nuclear Plant Nuclear Quality Assurance Program, section 15.2.3, require in part that nonconformances be identified and corrected.
Nuclear Division Procedure 600-1, " Control of Nonconforming Materials, Parts, and Components," section 5.0, requires in part, that corrective action be assigned and implemented for nonconformances.
Contrary to the above, corrective action was not assigned and implemented for Nonconformance Report (NCR) Number 86-024, which documented nonconformances associated with electrical conduit support for the "A" train of the hydrogen recombiner system.
This is a Severity Level IV violation (Supplement I).
s Pursuant to the provisions of 10 CFR 2.201, Portland General Electric Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which will be taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
