APPENDIX A NOTICE OF VIOLATION Portland General Electric Company Docket No. 50-344 Trojan Nuclear Plant License No. NPF-1 i

During an NRC inspection conducted from August 7 through October 1,1988, two violations of NRC requirements were identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions " 10 CFR Part 2 Appendix C (1987), the violations are list'd below:

A. 10 CFR 50 Appendix B, Criterion V, states in part, "Activities affecting quality shall be prescribed by documented instructions procedures, or i drawings of a type appropriate to the circumstances ar.d shall be ,

accomplished in accordance with these instructions, procedures, or  !

drawings."

Plant Safety procedure, PS-8-5, Revision 2 dated March 28, 1988,  !

"Ladders," requires ladders to be tagged with the user's name and date i the ladder is first used for a job, and that unattended ladders be ,

securely tied down or laid down to prevent equipment damage in the event  ;

of seismic activity.

Contrary to the above, on September 19, 1988, a ladder was in the vital  :

switchgear room of the control building not tagged, not tied duwn, l unattended and supported by switchgear ventilation ducting adjacent to i safety related switchgear,  :

Contrary to the above, maintenance request (MR) 88-6557 did not provide i appropriate work instructions to return to service reactor coolant loop i flow transmitter FT-424 after calibration on August 16, 1988. The [

instructions did not include appropriate methods for "Filling and Venting  ;

of Field Transmitters" as provided in Maintenance Procedure MP 2-3, f Revision 2 dated November 12, 1982.

"Reply to a Notice of Violation" and should include for each violation: (1) l t

  • 2-the reason for the violation if admitted.-(2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will  ;

be achieved. If an adequate reply is not received within the time specified '

in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken. Consideration may be given to extending the  ;

response time for good cause shown. '
