NOTICE OF VIOLATION Portland General Electric Company Docket No. 50-344 4

Trojan Nuclear Plent License No. NPF During an NRC inspection conducted from February 12 through March 25, 1989, violations of NRC requirements were identified.

In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations are lisima below:

10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

A.

Nuclear Division Procedure, NDP No. 600-1, Revision 4, dated February 10, 1987, entitled " Control of Nonconforming Materials, Parts, and Components", Section 1.0, states in part, "This procedure establishes measures to assure that nonconforming materials, parts, and components will be adequately documented and controlled...".

Contrary to the above, during the 1987 outage, nonconforming weld filler material was identified and documented on an NCR form, but the NCR was not processed in accordance with the established QA program procedures.

This is a Severity Level IV violation (Supplement I).

B.

Nuclear Plant Engineering Procedure NPEP 200-6, titled " Preparation of Engineering Drawings, Revision 3, dated May 29, 1985, requires in part that drawing change notices be incorporated in new or revised drawings within 90 days of issuance or whenever the number of drawing change notices exceeds five in number for a particular drawing.

1 Contrary to the above, drawing change notices 80 and 84 to Revision 8 of drawing E-22, the electrical fuse schedule, were issued on August 23, 1988, and October 12, 1988, respectively and were incorporated into Revision 9 of drawing E-22 on February 14, 1989, which exceeded the 90 day requirement.

Drawing change notices 80 and 84 dealt with safety related components of the auxiliary feedwater system and chemical and l

volume control system, respectively.

In cddition, drawing E-22 had 15 associated drawing change notices for revision 8 before revision 9 was issued.

This is a Severity Level IV violation (Supplement I).

. Pursuant to the provisions of 10 CFR 2.201, Portland General Electric Company is hereby requireu to submit a written statement of explanation to the U. S.

Neelear Regulatory Commission, ATTN:

Document Control Oesk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NE,C Senior Resident Inspector, Trojan, within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a

" Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an cdequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.

Consideration may be given to extending the response time for good cause thown.
