During an NRC inspection conducted on June 6-10 and June 14-17, 1988, violations of NRC requirements were identified.

In accordance with the 2

1 "General Statement of Policy and Procedure for NRC Enfor ement Actions,"

r 10 CFR Part 2, Appendix C (1987), the violations are listed below:

10 CFR 50 Appendix B, Criterion V, states in part, "Activities affecting quality shall be prescribed by documented instructions, procedures; or i

drawings, of a type appropriate to the circumstances."

l Nuclear Quality Assurance Program (PGE-8010), Section 5.2, states in part, i

"Activities affecting quality shall be accomplished in accordance with approved, documented instructions, procedures, or drawings."

During May 25 through June 4,1988 visual (VT"3) inspections were l

performed on ASME Section XI pipe supports utilizing the unapproved

guidelines in PGE memorandum JM-004-88M for acceptance criteria, j

As of June 10, 1988, written instructions or procedures were not l

1 established to prescribe the engineering evaluation process being t

performed on QC identified discrepancies of ASME Section XI pipe supports.

This is a Severity Level IV Violation (Supplement I).

2-Pursuant to the provisions of 10 CFR 2.201, the Portland General Elsctric Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice.
