Subject: NRC Inspection of the Trojan Nuclear Plant

This refers to the inspection conducted by Mr. R. C. Barr and Mr. G. Y. Suh of

this office on October 18 - November 21, 1987, of activities authorized by NRC

,

License No. NPF-1, and to the discussion of our findings held by Mr. Barr and

l Mr. Suh with Mr. Olmstead and other members of your staff periodically during

l the inspection.

l

Areas examined during this inspection are described in the enclosed inspection

'

i

report. Within these areas, the inspection consisted of selective

I examinations of procedures and representative records, interviews with

l personnel and observations by the inspectors. Within the scope of this

inspection two apparent violations of NRC requirements were identified,

as set forth in the Notice of Violation, enclosed herewith as Appendix A.

Your response to this Notice is to be submitted in accordance with the

provisions of 10 CFp 2.201 as stated in Appendix A, Notice of Violation. In

your response to the apparent violations, you should address the additional

concerns discussed in Inspection Report 50-344/87-37 for open items 87-37-01

and 87-37-03.

In the last six months, fo';r violations related to lack of procedural

compliance have been identified. Additionally, inspectors have identified

cases where procedures have not been corrected when the procedures have been

known to be incorrect and used by operators. In the exit of November 19,

1987, you identified additional actions being taken to insure this area of

weak performance; and you recognized that continued close self-critical

observation will be required.

We have also noted the recurrence of many similar events at your facility.

We are concerned that, on occasion, your implemented corrective actions have

not been far reaching enough to prevent event recurrence. We are encouraged

that you are taking actions such as Management Oversight and Risk Tree (MORT)

Analysis training to help correct this problem.
