As a result of the inspection conducted March 17-21, 1986, and in accordance, with the NRC Enforcement Policy, 10:CFR 2, Appendix C, the.following violations were identified:

A.
Technical Specification 6.11 requires that' procedures for personnel

radiation protection shall be prepared consistent with the requirements of 10 CFR' Part 20 and shall be approved, maintained, and adhered to for j
all operations involving personnel radiation exposure. The licensee'_s i..
Radiation Protection Manual developed pursuant to this specification I
requires in Section II.B.4.a. that a' determination of the medical' status-

of each person.who may be required to wear respiratory protective equipment will be made prior to his assignment to use it,y and repeated j
annually thereafter.Section II.H.2.c. states that, " Respirators will only be issued to personnel qualified _to use,the. respirator.
Qualifications consist. of.. 2) medical. evaluation..."

Contrary to the above, on April 16 and 18, 1986, a worker was issued a respirator for use whose medical reevaluation had expired'on February 28, i
1986, and another worker _was issued a respirator for use on April 30, 1986, whose medical reevaluation had expired on February 19,-1986. In each case, respirators were issued.and used pursuant'to radiation work permit requirements.
