Status Report of Corrective Actions for the October 14, 1987 Exercise of Offsite Plans and Preparedness for the Trojan Nuclear Power Plant Attached is a memorandum prepared by Federal Emergency Management Ager,cy f (FEMA) Region X on the status of corrective actions for the exercise conducted [
on October 14, 1937 for the Trojan Nuclear Power Plant. The memorandum, dated January 15, 1988, summarizes the status of corrective actions for 19 Deficiencies caserved at that exercise. This wac a joint, full-participa'.lon exercise for the State of Washington and Cowlitz County and the State of l Oregon and Columbia County. The attached memorandum is a followup of the t FEMA memorandum on preliminary findings and an exercise summary prepared by ;
FEMA Region X and transmitted to you on November 16, 1987. ,
i There are three Deficiencies for which FEMA believes there has been an [
inadequate response by the State of Oregon. The inadequacy of the Oregon !
response as well as statement of the Deficiency are detailed in the attached '
memorandum and are summarized below. The Deficiencies are numbered as they !
appear in the memorandum. !
l Deficiency #5 was based on the failure of the State of Oregon to desonstrate (
capabilities to fully staf f facilities and maintain staf fing around the !
clock through shift changes. The State submitted rosters and responded j that it may consider shift changes in the future. However, the State 15 ;
still not cowitted to resolving this Deficienev 5 nonths after the exercise ;
and after the completion of the 6-year cycle in which such exercise objectives are required to be Successfully demonstrated. (
7 Deficiency J6 was based on the inconsistency of the State EBS messages j with those of Cowlitz and Columbia Counties, which would have been broadcast ;
back-to-back by the EBS Station. The State failed to conttor the EBS l broadcasts or request copies of then to assure consistency and to avoid misinforming and confusing the public. The Governor did not follow the [
plan and delegated the broadcasting of the second and third EBS messages i to a staf f menber. Without providing the detail needed for confirmation, the t State responded that there was no discrepancy in the content of the messages. [
The State has agreed to review the coordination of the'EBS nessages and i make any appropriate changes, but its position is that the plan allows the Governor to designate any individual to make EBS broadcasts. (

) Deficiency H was based on the observation that during the exerr.ise, the
, State did not brief the media in a clear, accurate and timely manner. No j information was prepared and channeled to the Electronic Media Dissemination i Team or the Public Concern team that a major release of hanrdous radiation wel occurring at Trojan while the response organizations were briefing the medid and the pu'.ilic about the release. Telephone teams did not have current, timely or accurate infomation on the radiological emergency or status of response actions ared protective actions. The Media Production Team inaccuraf y l classified the event as J General Emergency and it was only because of an action by Washington State that this error by Portland General Electric and
- the State of Oregon was detected. The State response was that it nade a .
f conscious decision not to include infomation on radioactive releases in J their news releases in orer to keep rumor control simple. The State said j that it would evaluats onts policy and await another exercise before deciding
, whether to change it . r not. Also, the State believes that the quality of
- its infomation ma securate, and that with one exception, 'he flow of its i infomation was timely. FEMA believes that the State's response is inadequate.

) In light of these Deficiencies and the inadequacy of the response of the i State of Oregon FEMA is advising the Nuclear Regulatory Conmission (NRC) f that the Oregon offsite plans for Trojan do not currently satisfy the l requirements of 44 CFR 350 and NUREG-0654/ FEMA-REP-1, Rev.1. Thus, FEMA no longer has reasonable assurance that the Oregon offsite plans and preparedness for Trojan are adequate to protect the public health and safety
- in the Oregon portion of the Emergency Planning Zone (EP/\ around the plant.
- FENA Region X will request the State of Oregon to submit an acceptable schedule of corrective actions for the resolution of 1 a Deficiencies within 33 days from the date of the regional request. af Oregon does not take the necessary action, or the schedule is unacceptable, FEMA will invoke the provisions of 44 CFR 350.13 to begin the process of withdrawing approval of the State of Oregon's of fsite radiological plans and preparedness for Trojan, i Upon invoking 44 CFR 350.13, the FE'4A /mciate of rector of State and Loca' Programs (SLPS), through the Regional Director of FEMA Region X, will notify l the Governor of Oregon of the specific reasons for invoking 44 CFR 'l50.13.
- At the same time, NRC will be notified directly, if af ter four months fro 3 1
the date of that of ficial notification, the State has not corrected the
) relevant deficiencies or submitted an acceptable plan for doing sn, the j Associate Director of $LPS will withdraw approval of the State plan and i iriaediately infom the MC and the Governor of that determination. At that i time, FEMA would also publish notification of the withdrawal in the Federal i

As you know, within 30 days of the notificc. ion of withdrawal of approval of a State plan, any interested party may appeal the decision of the Associate
. Director of SLPS to the FEMA Utrector, according to the provisions of 44 CFR 350.13 (c).
The above actions apply only to the of fsite plans for the State of Oregon.

SUBJEi,T: Trojan Nuclear Power Plant Exercise Report, October 14, 1987 In accordance w!th the policies and particulars entablished in the SLPS memorandum of August fl. 19A3, 44 CFR 350.9, and Guldence Nemorandues EX-1 and PR-1, attached a e three copies of FEMA RX/RAC's report on the Trojan Nitclear Power Plant Plume Pathway Exercise that was held on October 14, 1987.
It in the Regior,'s and the RAC's annenument that thir exercise provided adequate opportunity to test response capabilities of the various organizatlonn.
It in our position, based upon this evaluation and overall assessment of preparedness issues, that the deficiencies outlined in t. ifs report do require a remedial drill which is scheduled for November 18, 1988 (Oregon State).
The recommendation fo corrective actions (those itean classified as deficiencies) have been discussed with tl'e exercise participants and concurrence in our evaluation and recommendations for these items has been indicated by all response agencies. We have requested f.hnt prompt remedial actions, as well as the normal corrective measure process, be token by the appropriate organivation. We will advise you of the status of corrective actions as appropriate.
