APPENDIX A NOTICE OF VIOLATION Public Service Electric & Gas Company Docket No. 50-354 Hope Creek Generating Station, Unit 1 License No. CPPR-120 As a result of the inspection conducted on October 21 - 25, 1985, and in ac-cordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

A. 10 CFR 50, Appendix B, Criterion III " Design Control" states in part:

" Design changes including field changes shall be subject to design control measures commensurate with those applied to the original design..."

The licensees NCR's 7967 and 7968 specified inadequate separation between redundant channel instruments PDT-N086A,B,C,D; N087A,B,C,0; N088A,B,C,0 and N089A,B,C,D. These NCR's were dispositioned as "use-as-is" based on the Project Engineering and Field Engineering conclusion that the instru-ments serve unrelated and non-redundant functions.

Contrary to the above, the licensee did not exercise adequate design con-trol in their disposition of these NCR's as "use-as-is" in that the in-spector determined that the instruments serve related and redundant functions.

This is a ' Severity Level IV violation (Supplement II).

B. 10 CFR 50 Appendix B Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings... and shall be accomplished in accordance with these instruc-tions, procedures or drawings...".

GE Drawing 791E421AC, Revision 3, dated August 28, 1980 indicated that the wiring connections for transmitters E51-PT-N050 and E51-FT-N051 should be such that:

positive terminal of E51-PT-N050 connected to terminal 5511

- negative terminal of E51-PT-N050 connected to terminal 5512 positive terminal of E51-PT-N051 connected to terminal 5514

- negative terminal of E51-PT-N051 connected to terminal 5515 Contrary to the above, on October 24, 1985, the inspector identified that the wires connected to terminals 5511 and 5512 were reversed and that the wires connected to terminals 5514 and 5515 were also reversed. l This is a Severity Level IV violation (Supplement II). l F512180308 851213 PDR ADOCK 05000354 O PDR OFFICIAL RECORD COPY IR HC 85 0004.0.0 12/13/85

Appendix A Pursuant to the provisions of 10 CFR 2.201, Public Service Electric & Gas Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
