On Decemher 14, 1931, while the reactor was in a shutdown condition, Class I hydraulic sheck suppressor CRS-219 was found to be inoperable. Since the reactor had been operated since this snubber was last successfully tested, the snuccer must be considered to have been inocerable when the reactor was at power.

This is a cegraded mode of LCO d.3.10 and is reportable per Fort St.

Vrain Technical Specification AC 7.5.2(b)2.

EVENT DESCRIPTION:

With the plant shutdown for maintenance activities, plant personnel observed cold reheat snubber CRS-219 to be inoperable. The lower eyebolt had been displaced from the mounting bracket allowing the snubber to swing freely from the upper mounting bracket.

CAUSE DESCRIPTICN:

While the exact cause of the occurrence cannot be determined, one of the two eyebolt cotter pins was found to be missing, which, along with pipe movement, could have allowed the displacement of the eyebolt from its mounting.

CORRECTIVE ACTION:

The eyebolt cotter pins were replaced with new and slightly larger cotter pins when re-assembling the snubber. A general inspection, following re-assembly, indicated that no other external physical damage had been incurred, and CRS-219 was returned to service as operable.

No further corrective action is anticipated or reauired.
