NPF-21 NRC INSPECTION REPORT 88-36 RESPONSE TO NOTICE OF VIOLATION The Washington Public Power Supply System hereby replies to the Notice of Violation contained in your letter dated November 4, 1988. Our reply, pursuant to the provisions of Section 2.201, Title 10, Code of Feder al Regulations, consists of this letter and Appendix A (attached).

In Appendix A, the violation, is addressed with an explanation of our position regarding validity, corrective action and date of full compliance.

ri A endix A During an NRC inspection conducted on October 10-14, 1988, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10CFR Part 2, Appendix C, (1987), the violation is listed below:

A. 10 CFR 20.202, "Personnel monitoring." states, in part:

"(2) "Radiation area" means any area, accessible to personnel, in which there exists radiation, ... at such levels that a major portion of the body could receive in any one hour a dose in excess of 5 millirem, ..."

-10 CFR 20.203, "Caution signs, labels, signals and controls." states, in part:

"(b) Radiation areas. Each radiation area shall be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words:"

"Caution "

"Radiation Area" ill Or "Danger" "

Contrary to the above, at 2: 15 p.m. PDT, on October 10, 1988, the Reactor Core Isolation Coolirig Pump (RCIC-P-1) Room, in which there existed areas with radiation dose rates of up to 10 mr per hour, was not posted, in that the signs had been moved such that they could not be read when entering to the room.

This is a Severity Level IV Violation (Supplement IV).

Valdit of Violation The Supply System acknowledges the validity of this violation. The reason for the violation is the radiation area posting was not restored after entry into the Reactor Core Isolation Cooling Pump (RCIC-P-1) Room.

Corrective Ste s Taken/Results Achieved

1. The posting was immediately restored to the correct position.
2. An evaluation was performed and it was determined that the physical configuration of the area (a narrow space between two airlock doors) was such that it was awkward to return the barrier to its correct position. Accordingly, on November 3, 1988 the barrier, and another barrier in a similar physical configuration, were reposted in a

-manner that does not require removal and reattaching the s-hook connector. The postings still provide for a highly visible barrier and also allow for more convenient access through the airlock doors.

The barriers have not been found out of place since the reposting on November 3, 1988.

3. Since early November, 1988 more indepth training during General Employee Training (GET) includes testing to verify the restoration of radiation area posting when entering or exiting a radiation area.

Corrective Action to be Taken P

1. A review of this Notice of Violation will be included in the Annual Radiological Refresher Training. This action will be completed by April 1, 1989 ~
2. The Health Physics/Chemistry Department is reviewing past Radiological Occurrence Report responses and actions. . Hhen the review is complete, a report (including any corrective actions) will be submitted to Plant Management. This action will be completed by December 31, 1988.
