NUCLEAR PLANT NO. 2, OPERATING LICENSE NPF-21 RESPONSE TO NRC BULLETIN 90-01: LOSS OF FILL-OIL IN TRANSMITTERS MANUFACTURED BY ROSEMOUNT

Reference:

NRC Bulletin 90-01: Loss of Fill-Oil in Transmitters Manufactured by Rosemount, dated May 9, 1990 Within 120 days after receipt of the subject bulletin, all holders of operating licenses for nuclear power reactors were requested to:

1) Identify all Rosemount Model 1153B,* 1153D and 1154 transmitters (except those manufactured after July ll, 1989) which are used in safety related systems or ATWS systems;

, 2) Determine if any of the transmitters identified in item 1) above are from the previously identified high failure rate manufacturing lots and replace, at the earliest appropriate opportunity, any of the high failure rate transmitters used in the reactor protection or engineered safety features actuation systems;

3) Review plant records for transmitters identified in item 1) to determine if any of them have already exhibited signs of fill-oil leakage and take appropriate actions;
4) Develop and implement an enhanced surveillance program for transmitters identified in item 1) to monitor for symptoms of loss of fill-oil; 9007 3. 7007V 900 1 1 2 PIIR ADOC4 0:-000397 0 F Dr.

5) Justify continued operation until the high failure. rate manufacturing lot transmitters used in the reactor protection or engineered safety features actuation systems can be replaced; and
6) File requested reports pursuant to the provisions of Section 182a, Atomic Energy Act of 1954, as amended,and 10 CFR 50.54(f).

The purpose of this letter is to provide the Supply System's response. to the .

above item.

As requested by the Reporting Requirements of Generic Letter 90-01, the Supply System hereby:

a) Confi'rms that items 1, 2, 3, 4 and 5 of Requested Actions for Operating Reactors have been completed.

b) At this point in time, only one of the suspect transmitters has exhibited signs of fill-oil leakage (HS-LT-26C).

This transmitter is a Rosemount Model 1153 Series D and was installed in the Hain Steam (MS) system. the approximate amount of time at pressure was 50 months. HS-LT-26C has been replaced (during RS outage) by a Rosemount transmitter manufactured 1989 that is not subject to the Part 21 Notice.

after July ll, The suspect transmitter will be shipped to Rosemount for testing and confirma-tion of failure mode.

c) The Supply System identified ten additional transmitters as being from the high failure rate lots. Those Model 1153 Services B & D transmitters are presently being utilized in the Hain Steam (HS),

Residual Heat Removal (RHR), and the Reactor Recirculation (RRC) systems. No Model 1154 transmitters were identified as being in use.

Subject to availability, the Supply System expects to replace the remaining Model 1153 Series B 8 D transmitters during the next refueling outage, scheduled to begin in April, 1991.

Safety related signals to scram, isolate, or perform other safety related functions do not rely on single transmitter Rosemount transmitter failures would be necessary origination'ultiple to prevent automatic actuation of a safety related function.

2. The Supply System will continue to monitor (trend) the remaining Rosemount Model 1153 Series B & D transmitters for loss of, fill-oil characteristics, and if identified shall review for reportability under the appropriate NRC regulations. Information consistent with item lb) above will be main-tained.

This completes our response to NRC Bulletin 90-01.
