NUCLEAR PLANT NO. 2 LICENSE NO. NPF-21 NRC MANAGEMENT MEETING REPORT 88-23 In the cover letter to the subject Management Meeting Report, the NRC presented a concern with regard to recent errors in the preparation of equipment clearances and requested that the Supply System respond to the issue within 30 days.

Accordingly, the purpose of this letter is to describe the actions taken by the Supply System to ensure an effective level of personnel safety during the performance of maintenance activities.

On May 17, 1988 the Plant Manager issued an Interoffice Memorandum (IOM) direct-ing that all electrical work (associated with switchgear, switching circuit interfaces, transformer and backfeed operations) be put on hold pending a complete review of the adequacy of associated Clearance Orders (COs) established to provide worker protection. The Stop Work was a direct result of a near miss on May 15, 1988 where an electrician received burns to his hand when arcing occurred on initial contact during an attempt to work a hot load side 4160V stab connection.

A Task Force of Plant Operations, Technical and Electrical Maintenance personnel was established at that time to conduct the reviews and screen all work packages associated with electrical switchgear work. This effort continued through the remainder of the recent refueling outage (R-3). In addition, electrical shop meetings were conducted on the ramifications of the near miss and an accident investigation committee was formed to evaluate the circumstances surrounding the incident.

On May 27, 1988 the Plant Manager issued a second 10M+hich underscored and clarified the responsibilities of plant personnel with regard to Clearance Order activities. The IOM also further emphasized the need to understand CO philosophy and follow the requirements of Plant Procedure 1.3.8, "Equipment Clearance and Tagging", to ensure that plant activities are conducted in a safe environment.

Page Two NRC MANAGEMENT MEETING REPORT 88-23 Currently, a separate committee continues to perform the functions of the pre-viously described Task Force for scheduled maintenance activities. The new group, consisting of a Control Room Supervisor, Shift Technical Advisor and Electrical Maintenance Foreman, is responsible for the following:

~ Reviewing Maintenance Work Requests and preventive maintenance tasks for adequacy of work scope definitions.

~ Assuring plant configuration for requested work.

1 e Assuring safety by reviewing Clearance Order boundaries to assess Clearance Order adequacy for the work to be performed, prior to Control Room review and issuance.

The performance and effectiveness of the group will be monitored and adjustments made accordingly.

As stated by the Plant Manager in the May 27, 1988 IOM, preplanning activities cannot be overemphasized in our efforts to improve both work efficiency and safety conditions. The goal of the Supply System is to work safely and provide quality work products in a timely manner.
