IE Bulletin 82-01, Rev. 1, Alterations of Radiographs of Welds in Piping Subassemblies, dated May 7, 1982 The above referenced IE Bulletin required applicants for an Operations License or holders of Construction Permits (group 1, table 1) to indepen-dently reverify that the examined welds of the subassemblies fabricated by AP&E are acceptable for plant service.

Attachment (A) provides our interim report describing the findings and corrective actions to be taken, signed under affirmation under the pro-visions of Section 182a, Atomic Energy Act of 1954. A final report will be submitted to your office upon completion of all corrective actions.

ATTACHMENT A INTERIM REPORT SUPPLY SYSTEM NUCLEAR PROJECT NO. 2 DOCKET N0. 50-397 LICENSE NO. CPPR-93 IE BULLETIN 82-01, REVISION 1 RADIOGRAPHS OF WELDS IN AP&E PIPING SUBASSEMBLIES

Background

At the Supply System's request, che site Construction Manager (Bechtel Power Corporation) Level II NDE Technician performed the independent reverification of the AP&E radiographs.

All AP&E supplied radiogt 'phs were obtained from the Supply System's QA Records Vault and the radiographs of pipe welds for material }" in thickness or less were reviewed by a Level II NCE Technician in the following areas.

e Penetrameter images were examined for the required sensitivity (2-4T).

e If the penetrameter image showed the required sensitivity, the film  !

was examined for alterations by the method described in IE Bulletin No. 82-01.

e If the penetrameter image did not demonstrate the required 2-4T sensi-tivity, or if the film was altered to artificially enhance the penetra-meter image, the film was reviewed for weld quality to ASME Section III requirements.

l e The reader sheets for all film, with and without the required sensitiv-ity, were reviewed to verify that the film had been read and accepted by a Level II or III Interpreter, the Authorized Nuclear Inspector (for ASME III welds only) and the customer's representative.

Review Results A total of 1,682 welds were reviewed with the following results.

Number of welds with less than 2-4T sensitivity 96 Number of welds with altered film 15 Number of welds with incomplete signatures on the RT reader sheets 51 Number of welds which did not meet ASME Section III weld quality 1 Of the 96 welds rejected with less than 2-4T sensitivity, 92 were for no T hole visible, 2 for the wrong size penetrameter,1 for no 4T hole visible, and 1 for no penetrameter outline visible.

For the 15 welds rejected for altered film, 3 were for alterations Dy scrat-ches,11 for alterations by pencil, and I with a cut hole making a light leak.

There were 51 welds with incomplete signatures on the RT reader sheets, and in every case, only the Authorized Nuclear Inspector's signature was missing.

Please note that it is neither a Code requirement nor a WNP-2 specification requirement that the ANI sign the RT data sheet.

In regard to the one (1) weld which did not meet ASME Section III weld quc.1-ity, the defect is very small, difficult to interpret, and therefore, is conceivably a legitimate oversight rather than an intentional disregard of Code requirements.

Current Status of Corrective Actions The WNP-2 Project has instituted the following actions to correct the discrep-ancies identified as a result of the independent reverification of AP&E radio-graphs for materials of }" or less.

The companion films held by AP&E for the welds with less than the required sensitivity and altered film will be reviewed by an independent Level II NDE Interpreter to ascertain the film's acceptability. If the film is accept-able, the AP&E film will be substituted for the Supply System film as the radiographs of record for the Project. In the event the AP&E weld film is not acceptable, we will re-radiograph the weld views with questionable exist-ing radiographs and evaluate the new radiographs in accordance with ASME Section III weld quality requirements, as well as WNP-2 specification require-ments. Any identified weld defects outside Code or specification allowables will be documented and repaired in accordance with site approved procedures.

Also, the Project will advise AP&E of the RT reader sheets without ANI signa-tures for their information and action. The existing records need not be revised, however, as this is neither a Code nor a WNP-2 specification viola-tion.

In regard to the one (1) weld with an identified weld defect outside Code allowables, a nonconformance report has been issued to document the defect and sent to the Architect Engineer for disposition.
