Thisnotification ismadepursuanttoCookNuclearPlant'sNPDESPermitMI0005827, PartII,A.5.OnNovember8,1997,duringcontinuous chlorination oftheServiceWaterSystems,plantpersonnel inadvertently discharged aportionofchlorinated ServiceWatertoOutfall002forapproximately ninehourswithoutperforming therequiredsamplingandanalysisforTotalResidualOxidant.Thesesamplesweremissedduetoavalvelineuperrormadeduringthestartupofcontinuous chlorination afterashortshutdownforgeneralmaintenance ontheCirculating WaterSystem.Shortlyafterthelineupwascorrected, asamplewastakenfromtheOutfall002discharge samplepointat10:15a.m.onNovember8,1997.Thesamplemeasured0.07ppmTotalResidualOxidant(TRO),whichexceededthe0.036ppmmaximumdischarge limit.Thecondition wasdiscovered duringatourbyplantpersonnel atwhichpointthechlorination systemwasshutdownNovember8,1997at-09:00a.m.Shortlyafterthediscovery, thevalvewasrepositioned topreventdischarge fromOutfall002anddirectalltheServiceWatertotheOutfall001,wherecirculating waterdilutionispresent.Continuous chlorination resumedNovember8,1997at09:45a.m.Abriefingwasheldbytheplantpersonnel involvedwithchlorination toemphasize theimportance ofcheckingtheCirculating WaterSystemvalvelineuppriortocommencing chlorination asrequiredintheirprocedure.

Thechlorination procedure includesstepstoensurethatpropervalvelineupshavebeenperformed toavoidNPDESexceedences.

Mr.FredMorleyPage2November12,1997TheServiceWaterSystemflowshavebeencalculated tobeapproximately 24.MGDperunit.Thisisaslightportionofthenormalcirculating waterflowof1415MGDforOutfall002.Thisdischarge causednodetrimental effectstotheenvironment duetothesmalldischarge flowandthelargedischarge piping(18'iax0.5mileslong).Normalwatercolumndemandforchlorinewouldhavereducedtheconcentration to<.001ppmbythetimetheflowreachedLakeMichigan.
