Dear Mr. Morley:

RE: Cook Nuc3.ear Plant NPDES Permit No. MI 005827 This notification is provided pursuant to PART II, Section A.S. of NPDES Permit No. MI0005827. On June 6, 1992, at 1350 hours0.0156 days
0.375 hours
0.00223 weeks
5.13675e-4 months
EDT, duping intermittent chlorination of the plant service water systems, a grab sample of Outfall 002, circulating water discharge was analyzed for total residual chlorine (TRC).

The analysis'ndicated a TRC concentration of 0.76 mg/l.

The sodium hypochlorite flow to the systems was immediately reduced. At 1404 hours0.0163 days
0.39 hours
0.00232 weeks
5.34222e-4 months
, a grab sample analysis indicated the concentration of, TRC had decreased to 0.22 mg/l. At 1414 hours0.0164 days
0.393 hours
0.00234 weeks
5.38027e-4 months
, the TRC concentration had decreased to 0.03 mg/l.

The daily maximum value for TRC during the time of chlorination was 0.18 mg/l.

'\

A malfunctioning non-essential service water flow meter has been identified as a contributing factor to the event. The flow meter is scheduled for replacement prior to resumption of chlorination activities on the non-essential service water systems.

By copy of this letter, the Nuclear Regulatory Commission is notified pursuant to thc requirements of Appendix B, Part II, Nonradiological, Environmental Protection Plan, Section 3~20 Very truly yours, Donald L.
