Donald C. Cook Nuclear Plant NPDES Permit No. MI0005827 As required by NPDES Permit MI0005827, Part II. Section A.6. Spill Notification, this written notification is being provided.

The Cook Plant's service water system cools the main turbine lube oil reservoir. This water system eventually discharges to Lake Michigan via Outfalls 001 and 002. On June 17, 1998, at 0100 hrs, the service water system cooling the unit 2 main turbine lube oil reservoir was found to have a tube failure. The heat exchanger was isolated immediately to prevent further oil leakage into the service water system, and spill cleanup commenced.

A visual survey of the unit 1 discharge showed very small oil sheens (<'2" in diameter) that surfaced and broke up within 20 feet of the discharge. The unit 1 circulating water system was removed from service June 17, 1998, at 1359 hrs to prevent additional oil from reaching the lake. Further surveys later in the day verified that the small oil sheens were no longer present in the unit 1 discharge.

An inspection dive was conducted on June 19, 1998, to verify that there was no accumulated oil in the unit 1 discharge tunnel prior to restarting the unit 1 circulating water system. The unit 1 circulating water system was returned to service on June 19, 1998, at 2300 hrs.

It was initially estimated that approximately 900 gallons of Mobile DTE 797 oil leaked via this pathway over a 24-hour period. However, based on estimates of the amount of oil collected from the cooler itself, and by absorbents placed in the circulating water system, we conservatively estimate the amount of oil released to Lake Michigan to be 780 gallons.

The leaks were caused by corrosion of the tubes, and have been repaired. An inspection/preventive maintenance program has been established for the lube oil coolers using fiber optics and eddy current testing to monitorcorrosion, followedby

Mr. Fred Morley Page 2 June 25, 1998 corrective actions to prevent tube failure. The coolers will be eddy current tested and repaired if necessary, prior to placing them back in service.
