Dear Mr. Morley:

This notification is made pursuant to Cook Nuclear Plant's NPDES Permit MI0005827, Part II, A.6, Spill Notification.

On February 9, 1998, at 1405 hours0.0163 days <br />0.39 hours <br />0.00232 weeks <br />5.346025e-4 months <br />, a spill occurred when a sight glass on a rental plant heating boiler failed. Most of the boiler condensate leaked into the trailer that houses the boiler control room and spilled Into a temporary spill containment built under the trailer.

.However, some spilled orito an asphalt drive and down a nearby storm drain.

Approximately five gallons of boiler condensate containing approximately 5-18 ppb hydrazine and less than one ppm ethanolamine reached the plant's storm drainage system that discharges to Lake Michigan.

The heating boiler operator discovered the condition as steam leaked through the doors of the trailer. The boiler was immediately shut down and the site glass was isolated to stop the leak. About 200 gallons of boiler condensate was collected in the containment berm under, the boiler. Once the site'glass broke, the'condensate tumed to. steam, allowing some ..

condensate to escape through the doorways to the trailer and drain to a nearby storm drain.

Environmental personnel were notified immediately and responded by diverting the condensate from the storm drain and absorbing the excess with universal absorbents.

PEAS Operator A5 (Cathy) was notified of the event at 1720 hours0.0199 days <br />0.478 hours <br />0.00284 weeks <br />6.5446e-4 months <br /> on February 9, 1998.

The site glass has been replaced and we have instafied a plexiglass shield around the site glass to protect the operators. No further preventive action is deemed necessary.
