Re: Cook Nuclear Plant NPDES Permit No. MI 0005827 The following notification is submitted p'er NPDES Permit MI 0005827, Part I. Section A. 15 (c).

On August 31, 1995 it was determined that the discharge from Outfall 00C (heating boiler blowdown) exceeded the concentration of monethanolamine (ETA) that was submitted in our NPDES Permit application. The actual concentration of ETA in the heating boiler blowdown to be 630 ppm, whereas the average ETA was'stimated concentration reported in the NPDES application was 15 ppm-This incident occurred when a valve connecting the chemical feed system to the heating boiler recirculation line was aligned in the open position. The open valve allowed the chemical feed tank to empty into the recirculation line and into the boiler.

An estimated 42 lbs. of ETA were discharged to the forebay. In perspective, our NPDES application estimated the amount of ETA discharged to the forebay as 639 lbs./year. This value was based on heating boiler runs of 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> per day for 15 days per month, while the actual average runs have been less than two days per month in 1995. Therefore, the total amount of ETA discharged to the environment remains well below the estimated annual values. No adverse environmental or public health effects occurred as a result of this event;
