Donald C. Cook Nuclear Plant NPDES Permit No. Ml 0005827 This notification is made pursuant to Cook Nuclear Plant's NPDES Permit Ml 0005827, Part II. Section A. 6, Spill Notification.

On March 27, 1998 at 0820 hrs, a condensate drain valve located on the outside panel of rental plant heating boiler failed leaking a small amount of condensate on to the metal skirting surrounding the containment berm. The cracked valve leaked approximately 5 gallons of condensate past the spill containment and into the storm drain. This water eventually discharges to Lake Michigan via Outfall 001s. Although the discharge was minimal, Stormwater Outfall 001s discharge was analyzed for ethanolamine and hydrazine. The results indicated 22 ppb hydrazine and 3.38 ppm ethanolamine. We estimate that approximately 0.000000917 lbs. of hydrazine and approximately 0.000141 lbs. of Ethanolamine were released to the lake during the event. The discharge intermixed with the beach sand along Lake Michigan and will naturally break down in the environment. This spill does not pose a threat to the environment or public health or safety.

The heating boiler was immediately shut down to depressurize the system. The failed condensate valve was repaired. The storm drain catchbasin's contents were pumped out and properly disposed.

Ms. Margaret Fields of the MDEQ Plainwell Office was notified of the spill at 10:02 hrs on March'27, 1998. The protective shielding that prevents snow and cold air from the underside of the boiler trailer has been modified to aliow the containment berm to catch any future leaks that would occur if this valve were to fail again.
